This agreement is between SamuelJon Media LLC ("SamuelJon," "we," "us") and the participant(s) listed above ("you," "your family"). It covers one episode of A Seat at the Table - a filmed conversation series produced for Manny's Fund, in which bereaved families sit down and introduce the children they carry.
You reached out to be part of something meaningful, and we don't take that lightly. This form exists to make sure we are both protected, both clear, and both walking into this with the same understanding. If anything in here doesn't sit right, say so. We will talk it through.
By signing, each participant listed above grants SamuelJon Media LLC a license to record, photograph, edit, reproduce, publish, display, distribute, and archive their name, likeness, voice, image, and on-camera performance as part of this episode and the Episode Release Package described in Section 5. This license does not expire and does not require ongoing payment. We use the formal legal language "irrevocable, perpetual, royalty-free" because it protects the episode and the series from being disrupted by changes in circumstance - not because we would ever use your story carelessly or against your wishes. Section 7 describes exactly how and when you can request removal.
Minor children actually present: If minor children will be present during filming, appear on camera, or be audible on audio, the signing parent(s) are granting consent on their behalf for that appearance as part of this agreement. This covers only the children who are actually participating or present in the filming space - not every minor in your household. We will talk in advance about how to handle children in the space so that everyone, especially them, feels comfortable and is not filmed in a way that anyone objects to.
Other household members: If other adults may be in the home during filming, the family signing this agreement is responsible for letting them know that recording will be taking place before we begin. Anyone who does not wish to be recorded should not be in the filming area while cameras are rolling. We will not pressure anyone to participate.
By signing, you are giving SamuelJon Media LLC permission to bring our production crew into your home to film on the date and at the address listed above. Filming will involve cameras, lighting equipment, and audio recording, and you are consenting to all of it.
We will treat your home carefully, respectfully, and better than we found it. We carry general liability insurance for our productions. If damage to your property occurs as a direct result of our negligence, our insurance exists for that purpose and is the appropriate remedy.
SamuelJon Media LLC retains full ownership of all raw footage, the finished episode, and all production materials created during this project, including audio, visual, and edited content. The rights described in this agreement are a license from you to us - not a transfer of your personal narrative.
Your story - the memories you share, the words you speak, the experience you bring to that table - belongs to you. It always has. It always will.
For the purposes of this agreement, the "Episode Release Package" means the finished episode and all materials reasonably derived from it for promotional use. This includes: the full episode, clips, excerpts, still frames, thumbnails, trailers, promotional social media posts, and use in SamuelJon Media LLC's professional portfolio, reel, and website.
Permitted without additional approval: Publishing the full episode on mannys.fund; promoting A Seat at the Table via trailers, clips, and posts on any platform; and representing SamuelJon Media LLC's production work through the Episode Release Package in its portfolio, reel, and website.
Requires your explicit written permission: Any use outside the above, including licensing to third parties, broadcast, paid commercial use unrelated to this series, or any context that presents your episode in a setting you have not seen and approved.
You have our full and permanent permission to use the finished episode however you like for personal, non-commercial purposes - on your social media, your personal website, in a memorial tribute, shared with family, played at a service. No cost, no restrictions, no expiration.
The only thing we ask - because it helps the series reach more families - is a simple credit where you feel it fits: "Produced by SamuelJon Media LLC for Manny's Fund." Given the nature of this series and what these episodes carry, that credit is a request, not a requirement. Use it however feels right for your child and your family.
Before anything is published anywhere, here is exactly how it works:
Delivery: Within 21 days of completing the edit, we will deliver the finished episode to you for review by email link or private video file.
Review window: You have 14 days from delivery to watch it and send us feedback. If we do not hear from you within 14 days, we will follow up once by phone and email. If you remain unreachable after that follow-up, the episode will be treated as approved after an additional 7 days - a total of 21 days from delivery.
Edits: You are entitled to one round of edit requests. Reasonable edits include factual corrections, removal of a specific segment that causes significant distress, and changes to how your child's name appears on screen. Reasonable edits do not include re-shoots, complete narrative restructuring, or changes that would misrepresent the conversation that actually took place. We will always use good judgment and err toward your comfort.
On the day: Anyone in the room - including Sam, Stacey, or any family member - can call for a pause or a full stop at any time, for any reason, without explanation. We will stop. We will take the time needed. We will never pressure anyone to continue filming.
Postponing: If your family needs to reschedule due to illness, emotional readiness, or any other reason, let us know as soon as you can and we will find another date. No penalty, no pressure. If SamuelJon needs to postpone due to equipment, illness, or crew issues, we will give you as much notice as possible and reschedule at no inconvenience to your family.
The family signing this agreement is responsible for ensuring that any adults present during filming - grandparents, friends, neighbors, siblings old enough to object - are aware that recording is taking place before we begin. If someone enters the filming space who has not consented to being recorded, we will pause. We will not resume until everyone in the frame is comfortable or has moved out of the filming area.
Minnesota law permits audio recording with the consent of one party to a conversation. That covers the primary conversation. It does not resolve every practical situation that can arise in a private home, and we want to talk through all of that before filming begins.
SamuelJon Media LLC will store all raw and finished footage securely. Raw footage - meaning unedited material that does not appear in the finished episode - will not be used for any purpose beyond the editorial process. We will not share raw footage with any third party without your written permission, with one exception: service providers who are part of the production workflow (editors, colorists, sound mixers, cloud storage and backup services) may access footage as needed to perform their work. All such providers are engaged under obligations consistent with this agreement.
If, after the episode has been published, you would like the raw footage permanently deleted from our storage, make that request in writing and we will honor it within 30 days. The finished episode and materials in the Episode Release Package are part of our production record and are not subject to deletion on request.
By signing, each participant represents that:
What you share in the conversation is true to the best of your knowledge and belief, and you are not knowingly making false or defamatory statements about any living person.
Any personal photographs, videos, audio recordings, letters, artwork, or other materials you provide for possible use in the episode are items you own or have permission to share, and to the best of your knowledge are not subject to copyright or privacy claims by someone who has not consented.
Any music that may be audible in your home during filming is incidental background sound and not something you have arranged to record as a featured part of the episode. If you would like specific music included, let us know and we will handle appropriate licensing.
You are entering into this agreement of your own free will, without pressure, and with full authority to bind yourself and any minor children who will be present during filming.
This document is the complete agreement between SamuelJon Media LLC and the participants listed above for this episode. It replaces any prior informal understandings about this project. If any provision is found unenforceable, the remainder stays in full effect.
If there is ever a dispute, both parties agree to attempt mediation before pursuing legal action. That feels right for something built on this kind of trust. If mediation does not resolve the issue, the dispute will be governed by the laws of the State of Minnesota, and any legal proceedings will be brought in Hennepin County, Minnesota. Each party is responsible for their own legal fees unless a court orders otherwise.
Both parties signing below confirm that they have read this agreement, understand it, and are entering into it freely and willingly.
We are honored to be trusted with this. We do not take it lightly.
